How effective is stacking birth control methods?

My girlfriend and I have had 3 times. She is on birth control (oral contraceptive pill) and I use condoms with a spermicidal lubricant. I have never 'gone' inside of her, pulling out before I am done. She's 19, I'm 23. Does we have anything to worry about?
